Insurance

Both Doctors take PPO and Medicare, Anthem Blue Cross Blue Shield, Blue Shield of California, and United Health Care.

Only Dr. Shariar Farzad takes Aetna and Cigna.

No vision insurance accepted.

Payment

All co-payments and deductibles are due at the time of service.  This includes payment for any surgical procedures, which is usually taken care of in advance.

We accept cash, check, and Visa, MasterCard and Discover credit cards.

We do not accept American Express.

Cancellations

In the event that you need to cancel your appointment, kindly provide 24 hours notice. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.
